Clinic Details
Our Baseline Clinic sits at the foot of South Mountain and represents the early days of Mountain Park Health Center.
Through renovations and expansions over the years, the Baseline Clinic now offers all Mountain Park services, including on-site dental and pharmacy.
Renovations through 2015 will ensure we continue to meet the growing needs of this community.
WIC is available to women in Arizona who are pregnant and breastfeeding.
Postpartum women along with infants and children under the age of five who are at nutritional risk and who are at or below 185 percent of the federal poverty guidelines are also eligible for WIC.

When arriving at the WIC clinic, you will need to bring the following:
Proof of income ( latest paycheck, last years tax return, or a letter from your current employer).
Bring a proof of AHCCCS, TANF, Food Stamps, or Section 8 participation who meet WIC income guidelines if you have them.

I scheduled an appointment for 4 p.m. on July 24th 2018. I arrived at 4:09 and called at 4:07 to ensure that I was still going to be able to be seen today. Upon arriving at 4:10 I was advised that I would not be seen instead I am able to wait until 4:41 if the 4:30 appointment does not show. when asked to speak with the supervisor no one was available for me to speak with today. the customer service in nonchalant attitude is completely unacceptable at any customer facing position.
